Application to modify the lease

Your application pertains to a modification that you want to make to the lease and that was refused by the lessee, and you want the Tribunal to fix the amount of the rent or that the Tribunal renders a decision on the desired modification.

This application must be filed by the lessor within one month following receipt of the lessee's refusal.

Address of dwelling in question: Enter the full address of the dwelling concerned by your application.

Description of parties

Lessee: Person that rents the dwelling in question.
Lessor: Person who signs the lease in the capacity of lessor.
